Pre - Appointment Medical Intake
& Consent Forms (NEW!) 

To ensure my clinic can safely and successfully re-open following the Covid-19 pandemic, pre-appointment consultations will now be done online. It is essential that the following online forms are completed before you attend your appointments. This change will help me run my practice as smoothly and safely as possible.

Failure to complete and submit the forms before your appointment may result in me not being able to let you enter my studio, and I will be unable to carry out any treatment until the forms have been submitted (which will eat into your treatment time.) Please note, I cannot begin any treatment until you have submitted these details.

  • You are an existing client, but it has been 6 months or more since your last visit.

  • You are an existing client, but you are coming to see Louisa regarding new symptoms/pain/injury.
